Hospice At Home
SPDNS Home Care have been in partnership with St. Luke's Hospice in Basildon and Thurrock for over 15 years providing an extensive home palliative support service in Billericay, Wickford, Basildon and Thurrock areas . The service works collaboratively with the patient and family and with any other services involved in their care. Most of our patients never visit the Hospice but are able to experience the ethos of the Hospice in their own homes and benefit from specialist care and support. This service includes hands on care, respite support, night support and access to 24 hour advice and assessment, face to face if required.
The service is proud that it was rated as Outstanding by CQC in July 2019.
OneResponse service
The service ensures the co-ordination of palliative and end of life care, and avoids unwanted and unnecessary hospital admissions. The service ensures that the patient is at the centre of what the service does, improving the delivery and access to care for people with palliative and end of life care needs. The Team work closely with the long established Hospice at Home service, and other community end of life services. Calls are dealt with by healthcare professionals and may involve giving advice and support over the phone or making a "rapid response visit" to the home. These visits are either made by one of the OneResponse team or a District Nurse or other community colleague within 2 hours of the call being received. The service is available 24 hours a day 365 days a year.
